Perceived patterns over time form a mental infrastructure—or mind-set. Scientists call it a paradigm. Danger arises when our mental maps cease to fit the territory. The problem with mind-sets or paradigms is that we tend to see through them, and so the degree to which they filter our perception goes unrecognized. To counter this threat, individuals and organizations must build mechanisms (i.e. checks and balances) that cause them to question and update their mental maps.
